Your counter-notification must include the following information:

- A statement that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
- Your contact information, including your name, address, phone number, and email address.
- A statement that you consent to the jurisdiction of the federal district court for the judicial district where you reside (or where the website is located if you reside outside the United States) and that you will accept service of process from the person who submitted the original DMCA notice.
We will forward your counter-notification to the original complaining party and take appropriate action as required by law.
